Import from Rhinobase for Windows

Transfer selected patients from the old Rhinobase Windows database into Rhinobase Next. Use Inspect and Dry-run first; Apply writes to PostgreSQL.

Before Apply, take a project and PostgreSQL backup. The importer updates patients by legacy ID and replaces previously imported legacy visit rows for those patients.
